Challenges and solutions for tooth supported or implant based prosthodontics in the posterior zone
The prosthetic treatment plan must allow an ideal integration of esthetics, biologics and functional requirements on natural teeth and on implants. In case of implants the prosthetic treatment is usually more challenging due to the inevitable soft tissues changing after teeth extraction. In fact, the soft-tissue healing and management around implants represents a crucial aspect and must be considered also from a prosthetic point of view in order to conclude the therapy with excellent outcomes from an esthetic, biological and functional point of views. In this sense the prosthetic components assume an essential role in order to idealize the prosthetic integration and simultaneously achieve reliable and stable prosthetic results. Clinical situations require specific evaluations in terms of implant connection, prosthetic components and restorative materials and approaches. Nowadays, it is well known that the biological integration and stability around implants, can be significantly influenced by the correct use of modern prosthetic components. In this sense the use of not-removable prosthetic components allows to idealize the soft tissues healing and management around implant with significant advantage from a biological, biomechanical and clinical point of views. Moreover the selection and the management of the restorative materials represents a crucial aspect in order to achieve excellent restorative integration with a good reliability.

IDR Technique - The Key for Long-term Success in Compromised Sockets in the Esthetic Zone
The Immediate Dentoalveolar Restoration (IDR) is a surgical and prosthetical technique established to broaden indications for immediate loading on individual teeth. In this way, tissue losses with varied extensions are reconstructed in the same surgical session of implant placement and provisional crown installation, reducing the number of interventions and keeping predictability on esthetic aspects. The IDR protocol was developed more than 11 years ago from the need to minimize the treatment time and morbidity of reconstructive procedures used in these cases. In the lecture we discuss the scientific basis, the step-by-step technique, indications and bone biology. The IDR technique, which advocates minimally invasive surgery, flapless procedures, is presented as a viable and reproducible alternative.

Soft tissue volume around dental implants – importance and clinical considerations
Esthetic demands of patients being provided with implant-borne fixed reconstructions have increased over the years. On the level of the peri-implant tissues, objective parameters exist to evaluate the outcomes of implant therapy including 2D and 3D changes of the peri-implant tissues (Furhauser et al., 2005). In case of volume deficiencies on the buccal side of dental implants, soft tissue augmentation surgery has been considered an integral part of implant therapy (Thoma et al., 2014b). Most frequently, autogenous subepithelial connective tissue grafts are used to augment soft tissue volume and according to the literature, are considered to be the gold standard (Thoma et al., 2009, Thoma et al., 2014a). However, the harvesting procedure is classified as being difficult and may be associated with the risk of intra- and postoperative complications (Del Pizzo et al., 2002, Soileau and Brannon, 2006). Therefore, research activities have focused on alternative devices to replace autogenous tissue and thereby eliminating the harvesting procedure. The lecture discusses the importance of soft tissue augmentations around dental implants and reviews available techniques and future trends.
Mirela Feraru - Bichacho (ISRAEL)
Management of soft tissues and ceramic restorations interface in the esthetic zone: From planning to execution
Restoring the smile zone is a challenging task for any dental team, where a successful result requires the combination of structural strength of the restored tooth as well as the aesthetic integration with the adjacent natural teeth. The treatment of significant aesthetic defects in natural teeth and surrounding tissue requires an in-depth knowledge, clinical capabilities and the use of modern technologies and advanced materials, in order to achieve the optimal result of healthy tissue and aesthetic performance.
Through a series of clinical cases new approaches, concepts and techniques will be comprehensively presented, explained in detail and critically analyzed, where functional and esthetic predictable results in healthy surroundings are the ultimate goal.
Sascha Jovanovic (U.S.A.)
Vertical and Horizontal Bone and Soft Tissue Augmentation using GBR, Autograft, Bone Substitutes, and Growthfactors
This presentation will focus on current and new modalities for vertical bone, soft tissue, and sinus augmentation, and its indications, results and complications in patients with alveolar bone resorption and esthetic expectations. Evidence based procedures based on 25+ years of research supporting bone grafting using GBR-procedures, autografts and Bone Fillers will be presented versus new procedures using cell mediators like rh-PDGF and rhBMP-2. In addition, soft tissue mucogingival procedures with palatal grafts and substitutes to improve vestibular problems, keratinization, and thin gingival biotype in the anterior esthetic zone will be discussed with patient selection, grafting protocol, and temporization options.
Objectives:
- 1. Understand the optimal clinical treatment protocol for grafting with bone and soft tissue.
- 2. Recognize limitations of growth Factors grafting procedures.
- 3. Diagnose and treat mucogingival problems in grafting cases.
Tony Rotondo (AUSTRALIA)
Simplifying space appropriation in the interdisciplinary case
Many dental conditions can create problems of space appropriation, these can include; microdontia and macrodontia; treatments involving canine substitution; patients where attrition, erosion and abrasion have altered tooth form and situations where teeth are not present or are in incorrect position.
While space appropriation may be simple enough when managing a local problem, it becomes more complex as multiple teeth become involved. On occasions where an entire dentition is affected, visualizing appropriate tooth position is only part of the inter-disciplinary teams problem, anticipating inter-arch and inter-incisal relationships can make visualization and ultimately the orthodontic positioning of teeth in anticipation of restorative care, very difficult.
Managing this type of case will be discussed, with specific reference to concepts for managing space appropriation that utilize direct restorative materials, provisional restorations and other form altering procedures. By utilising these methods, members of the interdisciplinary teams are able to share a complete vision for a particular case.
Marius Steigmann (Germany)
Surgical & Prosthetic Soft Tissue Development
After the period of pure osseointegration and function a new era of implant dentistry has begun demanding aesthetic soft tissue outcome. For this new incision flap design and sutures are necessary. This kind of surgery must be adapted to the individual situation of each patient depending on his Biotype and the quality of the soft tissue (Mucosa or Gingiva). These factors determine the way in which we do aesthetic implant surgery today. Different tissues react different to injuries and have a different patterns off of healing. The different suturing techniques ad to this faze of soft tissue predevelopment. In addition to the special care in the surgery in the aesthetic zone prosthetic design of the emergence profile have to be considered as restorations around implants trigger different reactions and biological changes than natural dentition. This prosthetic analog or digital design of the prosthetic components lead to improved soft tissue aesthetic specially after bone graft procedures in the aesthetic zone.
In addition to surgical intervention is the creation of the specific emergence profile that is essential in the aesthetic zone. The emergence profile composed of 2 parts, the abutment and the subgingival part of the crown. The shape of abutment can be individually shaped so that it gives natural appearance and varies individually (depending on the depth, angulation and diameter of the implant). Sometimes it even dictates the implant position. From a surgical perspective, soft tissue height, position and thickness need to be diagnosed and corrected when needed. From the prosthetic point of view, the emergence profile has to be created to mimic the natural appearance and maintained over time in respect to the biological changes. The course teaches step by step how to be successful with implant prosthetics from single tooth, partially edentulous to full arch reconstruction.
The main purpose of this course is to explain, demonstrate and teach soft tissue development around implants in fixed single and multiple implant placement.
Clinicians will learn how to build treatment plans based on individual circumstances
They will learn to utilize modern prosthetic technology for reconstruction of soft tissue for single and multiple implants.
The participants will get to plan prosthetically and aesthetically (soft tissue aspect) the position of the implant.

Does implants connection design influence long term success ?
the last year’s implant design has developed dramatically to provide better prognosis and superior peri-implant tissue support. what was allowed in the past as implant survivor rate is no more accepted by the clinicians nor the patients. the deep understanding of the bone physiology, biomechanics and the technological development of the implant manufacturing allowed to introduce major upgrades to the implant design to deliver a reliable treatment for our patients. we will discuss the impact of the implant fixture- abutment connection development and its influence on the implant positioning and the long-term implant success rate.

Evita Andrikopoulou (Greece)
Evita Andrikopoulou graduated the dental school of Athens in 2012. She completed her 3 year specialization in prosthetic dentistry in 2016 at the University of Athens. During 2015 and 2016 she collaborated as prosthodontist at the undergraduate clinic of the university. She attended a lot of European and International Conferences and she has published citations at Hellenic and European journals. She specializes in restoring edentulous patients with implant supported fixed prosthesis, hybrid restorations or removable dentures.
Edentulous maxilla: treatment plan with implants
Restoring the edentulous maxilla with implants is a challenging procedure for the clinician due to the centripetal pattern of resorption. The pink addition of ceramic or composite material is always a challenge when it comes to treatment plan with a removable or fixed restoration. Therefore meticulous diagnosis is critical to obtain a predictable outcome. Several articles and reviews have already been published presenting the factors that a clinician should take under consideration in order to make a precise workflow. The purpose of this lecture is to summarize, in a simple and comprehensible way, all the necessary information and criteria that the dentist must have in mind in order to manufacture a functional and at the same time aesthetic restoration.
Dimitris Xigkas (Greece)
He received his degree in Dentistry with honors from the University of Athens Dental School in 2009. He is American Board Certified as of 2015. He has written articles for international and national journals. His clinical research in periodontal plastic surgery was awarded in Billy M. Pennel Research Competition; Southern Academy of Periodontology; USA. He in an active member of the American Academy of Periodontology and International Team for Implantology. His clinical practice is limited to periodontics and implantology.
Periodontal Plastic Surgery: Minimally invasive for maximum success.
In periodontics, numerous plastic surgery techniques and materials are being studied. Today’s society has increased esthetic standards, and exposed root surfaces are considered unacceptable to many. Furthermore, denuded roots are more susceptible to caries and hypersensitivity. The early root coverage procedures utilized the patient’s own tissue, which translates to multiple surgical sites, limitation in the extent of teeth treated per surgery, increased chairtime and morbidity. Drawbacks from these early techniques have motivated research to test new methods and materials. New materials used with minimally invasive techniques make root coverage procedures more attractive to patients, who now have the option of undergoing single versus multiple surgeries to cover numerous recession defects with increased predictability.
Alexandra Papanikolaou (Greece)

In modern dentistry, the evolution of restorative materials and adhesive protocols has influenced the approach in treating the posterior region with composites. In our daily dental practice the aim is to perform a biological, functional, predictable, long lasting restoration, maintaining the vitality of the tooth.
So composites can be used for posterior teeth either for small and medium –sized cavities, or for massive adhesive restorations with cusp coverage compromised by tooth wear and erosion. Their application can be realized with different techniques: direct, indirect, intraoral, extraoral.
Certain criteria that will be described during the presentation, can guide the clinician to the right choice of proper material and technique.
